Forms like these would be cleaner and easier for other sections too, such as suggestions, UBRs etc.
Maybe even allow players to create forms themselves for clan/event applications, though not sure how MyBB works with this kind of thing.

It's possible, but it would require someone with AdminCP to create the form itself. No other way around that unfortunately and that's for very good reason as I'm sure you can imagine.
