Sorry simple minded people but; I think the message they're trying to get across here is. The water where they live isn't clean. Animals do their business in the water and the people in that area have no choice but to drink it since they don't have a source of water income like we do ( Taps etc ). So what they're saying is, They don't have clean water where they live and it can lead to illnesses. But they have to drink it to stay alive.
