As for making GTA IV run faster, there are countless tutorials all over the web - it takes more than 1 minute to google the good ones out, so I'll leave it up to you.
Yes, reinstall will leave you with a fresh start. GTA IV modifications aren't added content like they are for Gmod or CSS, but they're interchangeable with the original game files - thus when you download a custom car, it replaces a car in the game. You delete the custom car without making backups of the original files = screwed.
Also, GTA IV eats up even more of your computers resources when you've got custom content installed.
