I agree with a timer like 1% every 5 minutes or something because if you aren't getting in to fights and say walk outside from your passive and get hit by a prop, shot, or nudged by a car. It sucks to have lowered health when no paramedic is on.

It might actually work best if this feature is implemented above 80% normally and then if there are no paramedics maybe more? All depends on what would work best.
