Then how do false bans etc still occur? Clearly what you're doing isn't working very well.

I used words like "most" and "likely" for a reason. We are human, we make mistakes. You can't expect perfect work from players who volunteer time making rulings on something as subjective as roleplay. If a truly false ban occurs, an unban solves that.

How many of our bans are legitimate vs non-legitimate? Honestly, look at the sheer amount of bans and blacklists we have and tell me how many of those were wrongly given, and out of those how many stayed? If the number of unfair/incorrect bans and blacklists is high than we have a problem. 

Right now it just looks like there are complaints over issues that dont even exist, at least not to the extent that players such as yourself are making it out to be. Do you even have a number to work with here?

Let's say you're falsely ToS banned and have no contact to players or staff, know that ToS bans are given out pretty lightly
How will you get unbanned?

We don't give out TOS bans lightly. If a user breaks TOS, they no longer get the service, hence, terms of service. Only something like a formality or proof that they are innocent of what they were accused of will let the banned user back into the server.

It's a video game roleplaying server we don't need intricate methods that deal with every edge case. If something comes up that should be handled it has been proven time and time again that it will be solved one way or another.

Again, it is incredibly rare that a TOS ban is false, and some people only come back because of ignored unban requests and not because they were innocent.
There is clearly no proof required for a TOS ban to be made in the first place which assists lying.
