Basically he made 2nd UBR to this ban : http://www.fearlessrp.net/showthread.php?tid=39217&highlight=Kula short time after it was denied : THis one http://www.fearlessrp.net/showthread.php?tid=40195&highlight=Kula
Admins claimed he was lying to them about permission to post 2nd unban request. For that they banned him from game. After 2 or 3 months some kind of "policy update" been made, and his IG ban was changed into permanent forum ban.
